    The inquest into the self-inflicted death of Emily Hartley has concluded with the jury finding a lack of professionalism at HMP New Hall, including in the implementation of suicide and self-harm procedures (ACCT), contributed to the 21 year old’s death on 23 April 2016.

    Relatives of a woman who killed herself and her disabled daughter after repeated harassment have settled a claim against Leicestershire Police. An action brought by Fiona Pilkington's mother and Francecca Hardwick's brother, himself a vulnerable adult, has been settled by a five-figure sum.

  • Kelly Hutchinson

    Kelly Louise Hutchinson was the first woman's prison death in 2006. Her death was the eleventh death at New Hall Prison since 2002. A demonstration was called by Pauline Campbell, mother of Sarah Elizabeth Campbell, 18, who died in the so-called care of HMP & YOI Styal, Cheshire, in January 2003.

    A review published by the Warrington Safeguarding Adults Board has concluded that the self-inflicted death of Laura Davis was facilitated by staff at the inpatient mental health facility, Arbury Court. Laura was 22 years old when she was found unresponsive on her bed on 20 February 2017.

    The grim pattern to Louise Giles's short life was set at the age of 13 when she took her first overdose. She tried to kill herself throughout her teens. Three months after being sent to Durham prison, she was found dead in her cell with a ligature around her neck. She was just 20.

    Rebecca Overy was just 18 when she was found collapsed with a ligature around her neck on 23 June 2013 in Annesley House, a privately-run adult secure hospital in Nottinghamshire. Rebecca had a history of mental health problems and self-harm, and had been receiving treatment in hospital since the age of 13.

    An investigation by the Independent Police Complaints Commission (IPCC) into the death of a "vulnerable" man in a police cell has found "staff failed" to protect him. Andrew David Sheppard, 22, was found dead at Newport Central police station on 30 September 2006.

    Gwynne Owen Evans was one of the last two men hanged in Britain. Medical reports released in 2017 showed that Gwynne, who was hanged in 1964 at the age of 24, had serious psychological problems. But his defence team made no attempt to enter a plea of diminished responsibility - a plea that, if accepted would have saved his life.

    In 1991, a 15-year-old boy from Great Yarmouth was sent to Feltham Prison in West London, about 200 miles from his home, for 188 days. Social services denied him permission to go to the funeral of his grandmother. Shortly afterwards, he was found dead, hanged from his barred window. He had been sent to there for setting fire to a shed.

    A woman who was serving an eight-week prison sentence for shoplifting was found dead in her cell. Anna Graven, 41, was found at Foston Hall prison in Derbyshire. Paramedics tried to revive her but she was pronounced dead a short time later.

    On 30 March 2014, Christine Case a 40-year-old Jamaican woman died at Yarl’s Wood immigration removal centre near Bedford. According to reports in the press she was heard calling for help and had complained of chest pains shortly before suffering a heart attack. The emergency services were called around but she was pronounced dead at  the centre at 8.47am.

    Claire Harper, 41, was detained by West Yorkshire Police in the early hours of 7 January 2018 and taken to Trafalgar House in Bradford. She was later found “unconscious and unresponsive” in her police cell and despite being rushed to hospital, doctors were unable to revive her.

The jury concluded that the timing of the Mental Health Act assessment was inadequate, and the ambulance service failed to initiate a risk assessment when arriving at the scene.

    A man who died after being tasered by police by a 50,000-volt stun gun was not breaking into the property as thought but visiting friends for a quiet birthday drink, a friend has claimed. Adrian Thompson became unresponsive after being bundled into a police car and taken into custody in Newcastle-under-Lyme, Staffordshire, having been stunned.
